Direct Stabilization Program

We offer direct needs assistance and culturally grounded support in various areas such as:

  • Food and essential supplies
  • Clothing and household items
  • COBRA insurance
  • Appliance and utility support
  • Rent or mortgage assistance
  • Assistive devices not covered by insurance
  • Home repairs to support safety and accessibility

Legacy 5155 & Last Wishes Program

We help individuals honor meaning, culture, and final wishes through tailored legacy-building experiences.

  • Final wishes
  • Rituals and cultural practices
  • Family travel for end-of-life visits
  • Memory-making and storytelling projects

Funds will be used to purchase supplies, cover travel, or coordinate services.


Bereavement & Grief Support Program

We offer grief support, healing circles, caregiver support, and remembrance events.

  • Individual and group bereavement support
  • Culturally responsive grief workshops
  • Community healing circles
  • Legacy and remembrance events

Community Literacy & Equity Program

We deliver free or low-cost classes, culturally responsive workshops, accessible training, and resources on:

  • Health equity and advocacy
  • Advanced directives and planning
  • Culturally responsive caregiving
  • End-of-life literacy
  • Community wellness and resilience 

These sessions are delivered in person or virtually.

Program Determination Clause: A Heart’s Key Communal Care Alliance reserves the right to approve or deny requests based on organizational capacity, mission alignment, funding availability, community need prioritization, and safety protocols. Items listed under ‘basic necessities’ are provided as examples and may be expanded or limited as needed to maintain program sustainability and equitable distribution of resources.
