Our current efforts include:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Developing care models rooted in dignity, cultural alignment, and trauma-responsive practices<\/li>\n\n\n\n<li>Building community partnerships and collaborative networks statewide<\/li>\n\n\n\n<li>Preparing operational systems, training structures, and resource pathways<\/li>\n\n\n\n<li>Creating our Community Council and Alliance Partner Network<\/li>\n\n\n\n<li>Securing funding and support to sustain long-term care programs<\/li>\n\n\n\n<li>Gathering community insights to ensure our services reflect real needs<\/li>\n<\/ul>\n\n\n\n<p>This phase is vital.
