Until then, we are actively building the foundation needed to serve families with dignity, equity, and care. <\/p>\n\n\n\n<p>If you would like to become a resource or be informed with updates and programs please <a href=\"http:\/\/www.aheartskeycca\/org-connect-with-us\">CONNECT with US<\/a><\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Direct Assistance Program<\/h2>\n\n\n\n<p>We offer direct needs assistance and culturally grounded support in various areas such as:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Food and essential supplies<\/li>\n\n\n\n<li>Clothing and household items<\/li>\n\n\n\n<li>COBRA insurance<\/li>\n\n\n\n<li>Appliance and utility support<\/li>\n\n\n\n<li>Rent or mortgage assistance<\/li>\n\n\n\n<li>Assistive devices not covered by insurance<\/li>\n\n\n\n<li>Home repairs to support safety and accessibility<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">Legacy 5155 &amp; Last Wishes Program<\/h2>\n\n\n\n<p>We help individuals honor meaning, culture, and final wishes through tailored legacy-building experiences.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Final wishes<\/li>\n\n\n\n<li>Rituals and cultural practices<\/li>\n\n\n\n<li>Family travel for end-of-life visits<\/li>\n\n\n\n<li>Memory-making and storytelling projects<\/li>\n<\/ul>\n\n\n\n<p>Funds will be used to purchase supplies, cover travel, or coordinate services.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">Bereavement &amp; Grief Support Program<\/h2>\n\n\n\n<p>We offer grief support, healing circles, caregiver support, and remembrance events.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Individual and group bereavement support<\/li>\n\n\n\n<li>Culturally responsive grief workshops<\/li>\n\n\n\n<li>Community healing circles<\/li>\n\n\n\n<li>Legacy and remembrance events<\/li>\n<\/ul>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h2 class=\"wp-block-heading\">Community Literacy &amp; Equity Program<\/h2>\n\n\n\n<p>We&nbsp;deliver&nbsp;free or low-cost classes, culturally responsive workshops, accessible training, and resources&nbsp;on:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Health equity and advocacy<\/li>\n\n\n\n<li>Advanced directives and planning<\/li>\n\n\n\n<li>Culturally responsive caregiving<\/li>\n\n\n\n<li>End-of-life literacy<\/li>\n\n\n\n<li>Community wellness and resilience&nbsp;<\/li>\n<\/ul>\n\n\n\n<p>These sessions are delivered in person or virtually.<\/p>\n\n\n\n<p><em>Program Determination Clause: A Heart\u2019s Key Communal Care Alliance reserves the right to approve or deny requests based on organizational capacity, mission alignment, funding availability, community need prioritization, and safety protocols.
